New patients and injury claims

Your first visit takes about an hour: history, exam, a plain explanation of what we found, and your first treatment if it is safe to start.

How it works

  1. Call or book

Call (239) 555-0158 and tell us if you were hurt in a crash, at work or in sport.

  2. Bring your details

Bring a photo ID and insurance card. After a crash, add your claim and crash report numbers.

  3. Exam and explanation

We take your history, examine you and explain what we found, including any referral.

  4. Your plan, written down

You leave with a written plan: visits, care and the exercises to start at home.

Your first visit

Wear clothes you can move in. Plan on about an hour.

After a crash

If you were in a crash, get initial care within 14 days, or PIP will not pay for your medical care. PIP pays 80 percent of reasonable medical costs, up to policy limits.

Without a finding of an emergency medical condition by a medical doctor, osteopath, dentist, PA or APRN, the limit is $2,500. We tell you what your insurer confirms.

Hurt at work

If you were hurt at work, tell your employer and get the claim number. Florida workers' compensation pays only for care the insurer authorizes first, and we confirm authorization before we treat you.

Chiropractic care under workers' compensation has a visit limit unless the insurer approves more.

If plans change

If you cannot keep an appointment, call us as soon as you can and we will find you another time.

Insurance and auto claims

  • Florida PIP (auto)
  • Med pay
  • Workers' compensation (insurer authorization first)
  • Aetna
  • Cigna
  • Florida Blue
  • UnitedHealthcare

We bill Florida PIP directly and open the claim with your auto insurer at your first visit. Bring your claim number and your adjuster's name if you have them.

For health plans, we check your benefits before your first visit and tell you what you will owe. Your insurer decides what your policy pays.

Payment options

We accept credit and debit cards, HSA and FSA cards.

New patient questions

What does PIP cover?

PIP is the part of a Florida auto policy that pays 80 percent of reasonable medical costs after a crash, whoever caused it, up to your policy limit. The limit is lower if no emergency medical condition is found. Your insurer decides what it pays.

Can I pay without insurance?

Yes. We offer time-of-service rates and take credit, debit, HSA and FSA cards. Ask the front desk for current rates. Our stated charge for a service is the same whether you pay or PIP pays.
